Facilitates changes in nursing care based on systematic assessment and evaluation ā¢ Functions independently and/or as a member of an interdisciplinary team to plan, deliver, or evaluate health care for a defined patient population ā¢ Determines nursing care requirements and develops programs for a specific patient population, with regard to patient condition and age ā¢ Provides leadership for direct patient care programs in area of specialization ā¢ Collaborates with nursing staff/nursing administration to monitor and evaluate the quality of patient care ā¢ Serves in a consultative resource capacity to staff by providing expert knowledge and role modeling behavioral skills that impact clinical practice and patient outcomes ā¢ Promotes health maintenance that spans the continuum of health from outpatient to inpatient settings and back to the home care arena ā¢ Provides advanced nursing care for individuals, families, groups, both within UI Health Care and community ā¢ Provides anticipatory guidance and counseling to patients/families. ā¢ Provides leadership for and participates in unit, divisional, departmental, and/or interdepartmental committees ā¢ Serves as change agent on unit, clinical, departmental and hospital levels ā¢ Assumes responsibility for administrative activities and special projects as delegated by the Clinical Director/Medical Director ā¢ Contributes towards the achievement of divisional and departmental goals as delegated ā¢ Serves in a consultative and resource capacity to other divisions, departments, and the community as needed ā¢ Contributes to professional sharing of expertise and nursing knowledge with others ā¢ Contributes to the development of the body of nursing knowledge ā¢ Participates in professional nursing activities in the department, hospital, community and professional organization ā¢ Performs other special projects as assigned.
