Job DescriptionCentraās PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. ThriveAP provides Family Medicine NPs and PAs with a structured transition-to-practice experience focused on clinical readiness, continuity of care, and long-term patient relationship building across diverse populations.
Responsibilities
History and physical examinations, including history of present illness, review of systems, past medical history, drug allergies, surgeries and hospitalizations. Enhancements will include a split flow model of care to expedite and enhance patient experience of care, an integrated EmPATH psychiatric patient space, super track/imaging/results lounge and improved connectivity and utilization of the ED observation unit.
