p>UNC Hospitals Department of Pharmacy has an immediate opening for a highly motivated per diem Clinical Pharmacist Practitioner (CPP) to support ambulatory care clinics within the Triangle West/Chapel Hill region of UNC Hospitals. As clinic needs and available budgeted time allow, the CPP may also contribute to select quality improvement initiatives, workflow optimization efforts, or practice advancement activities relevant to the assigned clinical area.

PurposeThe primary responsibility of the Clinical Pharmacist is to provide oversight and safety for all daily operations of the assigned patient care team, including management of assigned pharmacy staff, clinical support to patients, and ensuring that all prescriptions are dispensed in accordance with state, federal and board of pharmacy laws. Verifies the discontinuation of service for each patient by assigning the appropriate therapy outcome and deactivates the patient s chart, prescriptions and software profile as appropriate so that the Patient Support group is informed to discontinue future refills.

This role includes reviewing referrals for appropriateness (drug, dose, route, labs), completing new patient starts and refills, performing medication reconciliation and patient counseling, monitoring lab-dependent therapies, and coordinating care with providers and nursing. The clinical pharmacy staff are comprised of almost 40 individuals who are involved in numerous quality improvement initiatives, precept students and residents and represent our patients' best interests across the health system.
li> Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols: monitors drug therapy regiments for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose; assists with pharmacokinetics consult service and drug dosing per organizational protocol; reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed; reads, extracts, and interprets information in patient charts accurately; provides drug information to patients and their families; documents all clinical activities and interventions accurately and completely; participates in quality improvement and medication use review activities of the department.
