Working with current RN Navigator disease team to provide non-clinical support regarding scheduling, pre cert process, follow up calls and record retrieval for timely appointments. Work with CPN, Navigators, Social Work, Dieticians, Call Center, Providers, Radiology scheduling, chemo scheduling, and phone triage to support patient's needs.

The Patient Navigator, Surgery is an advocate and educator for the patient, as well as family members through the Surgery/cancer continuum.

Ability to maintain (or exceed) minimum quantitative and qualitative performance metrics and expectations defined for position.

li>Complete daily rounds on inpatients to ensure that expected essential behaviors are demonstrated, and essential duties are being done, as it pertains to best practices affecting patient experience, safety outcomes, and HCAHPS performance.

p>Speak to patients pending discharge in an effort to arrange their outpatient appointments.

Voorhees Emergency Department - Onsite position $17.50 - $18.50 hr, Long Term Temp.40 hours a week.
