Acts as patient educator, leveraging tools (e.g., EHR, on-line resources, written materials, MY Chart messages, etc. ) to effectively communicate information with patients regarding diagnoses, treatment options, disease prevention, safe health behaviors, self-care skills, and health promotion. + Deliver patient care within HRSA services scope and approved locations, encompassing patient homes, community-based sites, and providing home-based care when necessary and in adherence to defined services and safety protocols.
