Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources. 2. Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions.
