Provide professional nursing services, including but not limited to; assessment, triage of walk-in and telephone patients, case management, planning of care and provision of care supportive to or restorative of life and well being Manage chronic and acute health problems under protocol Counsel clients on management of their illness while listening to their concerns, fears, and complaints Conduct educational sessions for all patients Administer and monitor medications, treatments, injections, immunizations, and PPD testing, as needed, to assist in the provision of care Perform EKGs, blood glucose tests, and other procedures Perform venipunctures Dress wounds Perform emergency care and routine first-aid Provide on-going patient education, counseling, follow-up, and advocacy Act as liaison for patient in respect to their care with specialists, consultants, other provider offices, laboratories, and insurance companies Refer clients needing further assessment or case management of episodic and chronic medical, surgical, podiatry, optometry, dental, psychiatric and other health problems identified through assessment Supervise and support medical assistants Provide follow-up on identified needs from quality assurance Participate in case conferences and other team activities Travel to various ChesPenn locations as requested Maintain confidentiality of clinical records and information in accordance with HIPAA regulations. Administrative: Coordinate medical services to ensure quality delivery of care to adults, pediatrics, and women's health care patients Communicate all pertinent organizational information to reporting staff Oversees CLIA regulations Follow requirements for State and local reporting of reportable diseases.
