In addition to providing hands-on nursing care, RNs work and serve in a broad range of capacities including, but not limited to, crisis RN staffing throughout facility (excluding PACU and L&D), case management support, physician clinic support and discharge planning; which will support assessment, planning, prevention, teaching, care, and treatment of the patient and the patient's family. Works collaborately with physician clinic offices in the "Primary Medical Home Model" implementation and maintenance Works collaborately with multi-disciplinary teams on prevention and maintenance of disease processes for patients and families Demonstrates compliance with hospital safety policies and procedures.
