Responsibilities:Work directly with mothers and children and assist with connection to community resources; Provide support in recognizing cravings, urges, triggers; and provide skill building including relapse prevention planning; Follow up with mothers up to six months post discharge, and provide links to supports as appropriate; and document; Prevent and assist with crisis situations; help reduce potential for relapse; Engagement and coordination with integrated team members for continuity of care, including written daily report when on shift; Record all documentation (UDS, med administration, individual sessions, group sessions, interventions, collateral phone contacts, and as needed releases of information, emergency contacts etc), in the EHR for continuity of resident records; Scan all written documentation into resident EHR (outside provider Urine Drug Screen, discharge paperwork, important documents, etc.); Requirements: High School degree or equivalent plus a minimum of one year of experience in a Recovery-oriented role. The recovery-focused program provides parenting support, vocational counseling, case management services, peer recovery support, medication education and support, along with life skills while linking residents with mental health and substance abuse counseling.
