Assertive Community Treatment Team (ACTT) is an evidence-based, intensive community treatment model designed for adults with severe and persistent mental illness who may struggle with traditional outpatient services and are at increased risk for hospitalization, homelessness, crisis episodes, substance use, or incarceration. The QP works closely with psychiatrists, nurses, therapists, peer support staff, and substance use professionals to support members with symptom management, independent living skills, medication adherence, housing stability, employment goals, and community integration.
