Collaborate with reentry coordinators, case managers, probation officers, and healthcare providers to ensure integrated care Facilitate referrals to community resources including detoxification, residential treatment, outpatient services, employment, housing, and social supports Document clinical progress and maintain compliance with program and regulatory guidelines Participate in multidisciplinary team meetings, training, and community outreach related to jail diversion and reentry.
