Facilitate care management to arrange/coordinate an array of services such as medical, emergency, psychological, financial (representative payee), family consultation and court systems that will help the individual meet daily needs, remain safe, and optimize their functional abilities despite high risk behavior. Must be able to physically perform the basic life operational functions of climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, fingering, grasping, feeling, talking, hearing, and repetitive motion.
