GENERAL SUMMARY: The Care Management Social Worker is responsible for collaborating with health care team members to identify and plan for the complex and problematic psychosocial needs of the patient and family. Provides crisis intervention services to patients, families and staff; assists patients, families and staff in.
ol> Demonstrates ability to appropriately modify approach and procedures to meet needs of a diverse population served for the following age groups: • Neonate (birth - 28 days) • Infant (29 days - less than 1 year) • Pediatric (1 year - 12 years) • Adolescent (12 years - 18 years) • Adult (18 years - 65 years) • Geriatric (over 65 years). The emphasis will be on completion of psychosocial assessment and intervention discharge planning, knowledge of community resources, appropriate documentation in the medical record, and networking with other hospital disciplines and community agencies.
