p>As a member of our multidisciplinary clinical team, the School Social Worker supports students' academic, social, emotional, and behavioral development through direct services, consultation, and collaboration with families, faculty, and external providers. Hyde Park Day School is a full‑day, specialized educational program serving students ages 6-15 with moderate to severe learning disabilities who demonstrate average to above‑average intelligence.
p>Master's Degree from accredited school of Social Work or related field; Valid license/registration of permit to practice in state(s) of agency operation if required; Medicare/Medicaid/Insurance specialty preferred; Unless otherwise dictated by the state, at least one (1) year of professional social work experience (post-graduate) in a healthcare setting required; 2+ years preferred; Experience or education in grief counseling preferred; Discharge planning experience preferred. Performs the full range of clinical duties with an increased and deepened clinical capacity including the gathering and analysis of data, formulation of assessment and treatment plans, and implementation of the treatment for routine and complex cases. Develops and implements skills in providing appropriate services, increasing responsibility for the appraisal of the effectiveness of own work through reflection and completion of all outcome measures documentation, treatment plans, and IEP.
