p>7 Works collaboratively with other members of the health care team, including medical staff, patient logistic practitioners, pain and palliative care members, Hospice and other community agencies to provide support and ongoing planning for psychosocial aspects of care throughout the stay and discharge transitions. Develops and coordinates discharge plans for complex cases such as patients with difficult family dynamics, patients facing end of life issues, placements to extended care facility (ECF), acute long term care facility, dialysis, arranging home healthcare and DME.

In collaboration with patient, family a primary care provider, develops plan to address and manage issues which influence health care utilization including services for home as well as facilitates hospital-to-hospital transfers, hospice, extended care facility, acute rehabilitation and long term care facility placement. Provides a variety of direct services and clinical interventions in order to provide continuity of care and to help patients and families resolve socio-emotional problems associated with adjustment to illness, resource needs, mental health problems and a variety of life events and transitions.
