The behavior analysts, therapists, social workers, counselors, and psychologists at SpringHealth combine their expertise to deliver high-quality behavioral services for clients to live more positive, active, and social lives.
Ideal candidates are patient-focused, mission-driven individuals who will be responsible for providing therapeutic services to patients dealing with a wide range of issues by counseling them and helping patients and their families navigate the medical system, assessing and monitoring patients such as depression, anxiety, and eating disorders by counseling them on managing ongoing health conditions and making positive behavior changes. Wisconsin: Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is preferred.

p>Required Qualifications • At least 2 years experience in health care, including at least 1 year of experience in care management, managed care, and/or experience in a medical or behavioral health setting, and at least 1 year of experience working with persons with disabilities, chronic conditions, substance abuse disorders, and long-term services and supports (LTSS), or equivalent combination of relevant education and experience. Job Summary Provides support for care management/care coordination long-term services and supports specific activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential.
