p>Key Responsibilities: Provide direct palliative care, including comprehensive assessments, symptom management, and care planning in community settings Conduct history and physical exams, order and interpret diagnostics, and document findings accurately Develop and implement individualized care plans aligned with patient goals, values, and quality-of-life needs Collaborate with interdisciplinary teams, physicians, and caregivers to ensure coordinated and holistic care Facilitate advance care planning discussions, including goals of care and end-of-life decisions Support hospice services through face-to-face recertification visits and ensure compliance with regulatory requirements Work a schedule of four (4) 10-hour days per week Participate in on-call rotation one week every six weeks (telephone only) No in-person visits required during on-call periods.
