Essential Job Functions (EJFs): (35%) Case Management Functions-Provides complex and specialized social work consultation and comprehensive case management services to families who receive services or are seeking services from the CSHCN-SP, children carved out of the MCO service delivery model, traditional Medicaid and STAR health clients remaining in PCS, SSI, NBS, CLPPP, CHIP, CHIP Perinatal, Pregnant women on Title V or unfunded, Department of Family and Protective Services, Personal Care Services Program, and CPW programs.
