Health & Wellness Director (LPN/LVN) - $5,000 Sign on bonus

This position oversees the day-to-day healthcare operations of the community support services and directly supervises the health care given, assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident’s opportunity to remain in the assisted living environment. Order medications and oversee the resident’s medication schedule for residents receiving medication administration services, and assure that all medications prescribed are available by auditing the medication records and cart twice weekly and ensure proper documentation for medication assistance administration.
