Assists with psychosocial assessments for patients evaluated for and receiving solid organ transplant or mechanical device and other patients identified, as neededProvides social work interventions to patients and their families including risk screening on all patients admitted to his/her unit through chart review and interdisciplinary rounds, psychosocial assessments for those with identified risk factors (including victims of abuse, neglect or domestic violence), providing psychosocial support including crisis intervention, supportive counseling and grief and bereavement counselingHandles risk management issues, in close consultation with his/her supervisor and the Clinical ManagerDocuments social work activity in the electronic medical recordProvides social work coverage, as requested, to other clinical units or programs during co-workers absenceCompletes the following Administrative Functions:Completes Hospital and SW Division required in-service training programs and ongoing professional development activities including those designed to enhance knowledge and skills in protective workCompletes required Employee Health appointments within required timeframeAttends all appropriate Social Work Division meetings including, but limited to - SW Division meetings, complex case discussion, transplant staff meeting, etcSupervises social work students (MSW or BSW)Orients and provides ongoing peer support to new social work staffOther duties as assignedMINIMUM EDUCATION & EXPERIENCEMaster''s degree from an accredited school of social work requiredOr equivalent combination of education and experienceLICENSES AND CERTIFICATIONSLMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required orIf duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire requiredThe University of Rochester is committed to fostering, cultivating, and preserving an inclusive and welcoming culture to advance the University's Mission to Learn, Discover, Heal, Create - and Make the World Ever Better. ESSENTIAL FUNCTIONSIn collaboration with the members of the health care teams, coordinates discharge planning for patients who are determined to be at psychosocial risk, provides appropriate hand off to other social workers when patients are transferred to other units within the hospitalServes as a resource consultant to patients and their families and facilitates referrals to appropriate community agencies and hospital resources; may make home visits, where appropriate, to provide essential psychosocial assessments and social work interventions.
