The social worker provides psychosocial assessments, supportive counseling, emergent crisis intervention appropriate to setting, financial resource information, environment enhancements, advance directive planning and referrals to community agencies for clients and their caregivers/families in the client‘s home, mobile health clinic (MHC), or other settings. The CHI Social Worker provides professional services to clients and their families eligible for the Healthy Beginnings Services and families on board the Mobile Health Clinic, in order to meet identified psychosocial, emotional, financial and environmental needs.

What this job involves: The Strategic Sourcing Specialist serves as the dedicated category manager for Integrated Facilities Management (IFM) services supporting a specific client account, with complete accountability for category strategy, supplier management, and sourcing execution. What your day-to-day will look like: Serve as the primary subject matter expert and procurement authority for the assigned IFM category, owning end-to-end strategy, sourcing approach, supplier management, and governance while driving value creation and continuous improvement.

p>The Program Supervisor of Intact Family Services is a critical member of both the region’s child welfare team, as well as the agency’s broader leadership team, working collaboratively within and across programs to ensure a relentless focus on keeping children safe and supporting families.

li>Location: Home office is the Community Child Care Center of Palatine Township and provides support for Family Support staff across both the Center Based program in Palatine and the DeKalb/Boone/Grundy Home-Based program.

The Family Support Services Supervisor provides leadership to a team of staff focused on the educational, social, and emotional needs of the early learners and families being served by our family support programs within our Palatine Center and DeKalb, Boone, Grundy Counties and surrounding communities.

p>Provides a superior level of customer service and portfolio management in lending services for all existing and potential clients and in support of a team of specialists and relationship managers.

PRIMARY RESPONSIBILITIES: Under general supervision of the Director of Resident Services or designee, the Family Self-Sufficiency Specialist shall facilitate and coordinate a wide range of specialized human services and activities in the case management of participants enrolled in the Family Self-Sufficiency program, the purpose of which is to assist these families in becoming economically independent of welfare assistance within five years of signing a contract of participation. EDUCATION and/or EXPERIENCE: College Degree in Sociology, social work or a related field, with four years successful related experience in case management.

The PCS-FOS Senior Manager is responsible for providing exceptional client service and supervising the work products of other professionals on the client relationship team; developing new business and working with the business development and marketing team as opportunities move through the pursuit process; mentoring and developing the interpersonal and technical skills of the other client relationship team members; and responsibly managing the PCS-FOS practice through mitigating risk and managing the financial performance of their respective client portfolio.

The Family Support Specialist engages families of enrolled children in activities, practices, and services that support parents as the primary nurturers, experts, and teachers of their child in order to promote and sustain their child's academic learning and development and life success. This includes helping parents recognize and expect "quality education"; know how to access quality elementary schools; and learn how to advocate for a quality education for their child; Activities and strategies that support parents in achieving family and community outcomes.

p>ESSENTIAL DUTIES & RESPONSIBILITIES:

• Manages all Thresholds Supported Services for Veteran Families residential properties, subsidized and non‐subsidized, in accordance with all current funding requirements, such as but not limited to Housing & Urban Development (HUD), Veteran Affairs Supported Housing (VASH) or equivalent project-based voucher program, current release of the HUD 4350.3

  The Housing Locator, SSVF (Supported Services for Veteran Families) reports to the Team Leader, SSVF and is responsible for the development of affordable housing opportunities for Thresholds members by building relationships with landlords and identifying suitable rental apartment units for members.
