· Actively participates in the long-term care survey process by instructing staff in matters of conduct and disclosure, always maintaining a presence when the surveyors are on-site and directing the timely collection of information required by the survey team. *Accountable for Social Services practice, measuring outcomes to interventions, and departmental productivity. *Assures that a psychosocial assessment is completed for each patient/resident that identifies social, emotional, and psychological needs and strengths. *Ensures that patient/resident discharge goals are identified at admission and documented. Assesses each patient/resident for discharge. *Educate staff regarding the role of Social Services and the psychosocial needs of patients/residents and their families/significant others including the problems of aging and disability. |
