p>The Center of Excellence (COE) Case Manager is responsible for providing comprehensive case management services to individuals with a history of addiction and co-occurring disorders, particularly individuals with opioid use disorders (OUD), and providing specialized support to The Wright Center clinical staff.
Supportive services include but are not limited to; completing forms, coordinating and providing transportation related to securing temporary or permanent housing, assistance with moving, homemaking instruction, apartment monitoring, etc. The Housing Case Manager will be responsible for housing related supportive services with all community-based housing options/providers utilized by the Lackawanna County Department of Human Services.
Through compassionate engagement and strong communication, our care teams collaborate with members, providers, and community organizations to address the full continuum of healthcare needs, including medical, behavioral, and social determinants of health.
