ACT Case Manager
As a Case Manager on our Assertive Community Treatment (ACT) team, you’ll work directly with adult clients to help them identify and build the lives they want. You’ll collaborate with a dedicated team that focuses on areas like housing, employment, therapy, medical care, and substance use support.
In this role, you’ll:
- Partner with clients to create personalized service plans based on their goals and strengths.
