Coordinate the Resident Assessment Instrument and MDS processes for facility residents• Develop and maintain an interdisciplinary schedule for MDS assessments and care plan reviews• Ensure MDS assessments are completed accurately and within required timeframes• Establish and communicate Assessment Reference Dates in collaboration with the interdisciplinary team• Coordinate completion of required assessment sections by appropriate members of the interdisciplinary team• Review the medical record to ensure documentation supports MDS coding, resident conditions, treatments, services, and functional abilities• Monitor documentation for accuracy, completeness, consistency, and regulatory compliance• Work with nursing, therapy, dietary, social services, activities, and other departments to obtain necessary assessment information• Participate in daily clinical meetings, stand-up meetings, Medicare meetings, and other resident review processes as assigned• Help identify significant changes in resident condition and determine whether an assessment or care plan update is required• Coordinate with the Director of Nursing or designee regarding physician orders, clinical reports, and resident changes• Ensure the care plan reflects identified resident needs, preferences, risks, strengths, and measurable goals• Facilitate interdisciplinary care plan meetings and encourage resident and responsible-party participation• Monitor Medicare eligibility, skilled coverage, certification requirements, and supporting documentation• Help ensure continued-stay documentation is completed accurately and within required timeframes• Support accurate clinical reimbursement and Patient-Driven Payment Model processes• Review assessment information for potential coding discrepancies before submission• Track MDS submissions, corrections, modifications, accepted assessments, and completion deadlines• Complete regular audits of MDS coding, care plans, and supporting clinical documentation• Report identified trends, risks, and audit findings to nursing leadership and the Quality Assurance and Performance Improvement Committee• Provide MDS, documentation, and care-planning education to clinical personnel as needed• Stay informed about changes to MDS, RAI, Medicare, Medicaid, and clinical reimbursement requirements• Assist with documentation preparation for regulatory surveys, audits, and medical record reviews• Maintain resident privacy, confidentiality, and protected health information• Perform other duties related to the position as assigned. This includes coordinating assessment schedules, reviewing supporting clinical documentation, monitoring Medicare requirements, participating in care planning, and helping ensure services are accurately reflected for clinical and reimbursement purposes.
