</h3><h3>Duties and Responsibilities</h3><h4>Leadership & Operations</h4><ul><li>Provide overall direction and management of home health and employee health departments </li><li>Develop and implement policies, procedures, and strategic initiatives aligned with organizational goals </li><li>Lead interdisciplinary teams including nursing, therapy, and employee health staff </li><li>Collaborate with executive leadership, HR, clinical departments, and external partners </li><li>Serve as liaison between home health services and internal departments </li><li>Communicate effectively with patients, families, staff, and regulatory agencies </li></ul><h4>Home Health Program Oversight</h4><ul><li>Oversee delivery of skilled nursing, therapy, and support services in patient homes </li><li>Ensure patient care meets quality, safety, and regulatory standards </li><li>Manage admissions, care planning, coordination, and discharge processes </li><li>Supervise clinical staff including hiring, training, and performance evaluations </li><li>Monitor patient outcomes, satisfaction, and care coordination </li><li>Ensure accurate documentation and compliance with reimbursement
