p>PURPOSE AND SCOPE: The Kidney Care Specialist (KCS) supports patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) by driving early education adoption, care navigation, and engagement through a combination of field-based physician and community outreach and virtual patient support.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provide virtual care navigation to CKD and ESRD patients regarding kidney disease progression, treatment options (including home modalities and transplantation), and self-management strategies using approved digital platforms.
The Care Team Specialist (CTS) is a care coordination role within the Certified Community Behavioral Health Clinic (CCBHC) model responsible for supporting timely access, engagement, and continuity of care for individuals receiving mental health and/or substance use services. Identify existing care team members, including medical, behavioral health, substance use, social service, and natural supports, and when such supports are not yet in place, identify and facilitate connections to appropriate resources and care team members.
