The Outpatient Liver Transplant Social Worker provides psychosocial assessments, crisis intervention, care and resource coordination for patients and their families before surgery and after discharge after transplant. This position works in collaboration with Inpatient Liver Transplant Social Worker, Transplant Coordinators, Psychiatry, Financial Counselors and Medical Providers.

It's helping families grow by making each delivery special, conveying a difficult diagnosis with a compassionate touch, going above and beyond for a patient's needs, or giving a high five when a patient beats a disease or conquers a personal health challenge.

ul> Must possess an active, unrestricted Nebraska license in one of the following: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
