The School Social Worker, under the leadership of the Managing Director of Related Service Providers and in collaboration with case managers, teachers and other related service providers, will use effective strategies to serve the needs of students. Social Worker: The Social Worker travels to patients homes, nursing homes or wherever the patient calls home to provide short-term crisis intervention and individual or family counseling and support as determined by the social workers assessment, family input, and the IDT plan of care.
