Job Summary and Qualifications The Transplant Social Worker will assist in the facilitation of care across the continuum for transplant patient care needs with a focus on the following:What will you do in this role:Perform biopsychosocial assessment of the patient to meet their healthcare needs; obtain information from the patient based on assessment; diagnose, provide therapeutic interventions, and make referrals as appropriate; provide supportive counseling to aid adjustment to patient's illness.
You will provide emotional support to patients and family members during complex and/or emotionally charged issues, such as end-of-life, terminal illness notifications, death notification (with physician), and crisis interventions. p>Role Description: The Medical Social Worker is a key member of Oak Street Health Centers, contributing a unique and valuable perspective to care teams to support the psychosocial needs of our patients.
Educate patients on supportive care options (including advance care planning and end-of-life care) and provide goals of care counseling, support, and advocacy at all stages of the patient journey.
To help the patient and his/her family understand Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) and renal replacement therapy, to counsel with the patient and family in working out their acceptance of the patient's medical problem and methods of modifying ways for more satisfying relationships, to provide a psychosocial summary to aid in the preparation of the interdisciplinary patient plan of care, to act as a resource person and liaison with community agencies, to provide casework and group work services to assist the patient and family, to evaluate the patient's optimum rehabilitation potential, to provide education regarding psychosocial problems of renal disease to members of the health care team and the public. Provides a psychosocial summary to obtain the patient and family history to determine strengths, weaknesses, and coping patterns; identify problem areas; and identify feelings about transplantation and possible transplant donors; and identify rehabilitation potential.
