A critical component of success, you proactively build and maintain meaningful relationships with professionals who interact with seniors and their families involved in guiding senior care decisions. Plan, develop, organize, implement, evaluate, and direct the marketing programs and activities to maintain and increase census and to provide the public with information relative to our community, its programs, services, and practices.
Through education and outreach, the CHW will assist clients/patients in overcoming barriers to managing their chronic health conditions and will refer them to self-management programs when appropriate in partnership with the clinical team. Attend regular Health Promotions team & GHWIC Strategy-related meetings, attend weekly/daily clinical team huddles, and attend GLIIHCs monthly all-staff meetings.
