*Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
*Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
*Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
*Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs.

*Maintain good working relationships with center employees, meeting with administration and nursing staff to collaborate and plan.
*Assure a comprehensive Psychosocial Assessment is completed for each patient that identifies social, emotional, psychological needs and strengths.
*Educate patients and families regarding their rights and responsibilities, health care decision making/advance directives, effective problem solving, and available community resources.
*Provide therapeutic interventions to assist patients to cope with their transition and adjustment to a nursing center and to address behavior or mood problems
Qualifications: *Bachelor's degree in social work or human services required.
*Must possess any certifications/licensures as required by state of employment to practice in long-term care.
