Bachelor's Degree in health services, public health, sociology, psychology or related field (And) Three (3) Years Work Experience with patient navigation, care management, or any kind of direct service provision to patients in a community health/outreach, and/or health care setting plus a bachelor's degree in health services, public health, sociology, psychology or related field.
