As a Community Health Worker, you will be assessing, planning, implementing, monitoring, and evaluating actions to meet with clients health and human services needs, so that they can adopt health behaviors leading to self-sufficiency. Working closely with school personnel, healthcare providers, and community partners, the CHW helps connect individuals to healthcare, behavioral health, insurance coverage, and community resources while promoting wellness and addressing social determinants of health. In this role, you will serve as a trusted connection between students, families, schools, healthcare providers, and community resources to improve access to care and support positive health outcomes.

p>Current Certification as a Community Health Worker under a CHW Training Program Pathway via completion of an MDHHS-approved Community Health Worker Training Program or Planned Certification as a Community Health Worker through the CHW Work Experience Pathway requiring verified completion of 1000 hours of experiential learning with the last three years and a commitment to complete the CHW Training Program Pathway noted above.

The Community Health Worker will be responsible for encouraging patient and provider shared decision-making, promoting healthy patient behaviors that reduce risk, improving medication adherence, furthering patient self-management skills, and linking the clinical care provided by the Health Center with community-based social supports.

The Community Health Worker (CHW) engages in collaboration with multiple team members as part of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of social determinants of health needs in the presence of other comorbid physical and behavioral health conditions.

  The Community Health Worker (CHW) plays a vital role in improving the health and well-being of the population served by assisting high-risk families with accessing available resources and providing education relevant to the community program’s focus.

This position will work directly with patients and their representatives to assist in the delivery of six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services.
