The Virtual Primary Care Nurse Practitioner works in collaboration with a dedicated care team to provide evidence-based, primary care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients 18 years of age and older. Virtual Care Team members must meet minimum requirements for remote care delivery, including broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelines .
