WHAT YOU MUST HAVE;
- Master of Social Work or a Master’s degree in a related field and less than five (5) years of work experience in the field; a person with a Bachelor of Social Work or a related undergraduate degree with more than five (5) years related work experience; or a person with nine (9) years of experience in the field and no degree would qualify for this position.
JOB SUMMARY AND QUALIFICATIONS:
Case Manager will assist with services to individuals and families experiencing homelessness who have been approved for RRH or Joint RRH/TH services In tandem with other members of the VOA Home team, the Case Manager will assist clients in accessing medical and mental health services, emergency and long-term housing, substance abuse services, and social services.
The CCM oversees the effective coordination of services and manages issues in the following main areas: admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education, payor relations and total fiscal management. The Certified Case Manager (CCM) serves as a key member of the interdisciplinary team and actively manages and directs resource utilization to achieve the highest quality outcomes during a patient's rehabilitation experience.

You will only see FOUR Hospice patients per day and will manage caseload of terminally ill, end-of-life patients, delivering skilled nursing care in home and facility settings, supporting patients and families throughout the end-of-life process . Collaborate with Physicians to regularly titrate medications and update individualized care plans while simultaneously working with IDT team in developing comprehensive, patient-centered plans of care .

The RJ Case Manager Specialist Lead shall provide case management services with referred individuals and their families while also implementing appropriate restorative justice practices.
WHAT YOU SHOULD HAVE FOR THIS ROLE:
- Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field, can be within four (4) months/one semester of obtaining degree; up to three (3) years of experience.
