INTRODUCTION:. Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, child care, and housing counseling.

The Care Manager will promote quality cost-effective outcomes managing care needs through the continuum of care utilizing effective verbal and written communication skills and a consumerism approach through education and health advocacy to members serviced.
Establishes a documented patient centric care management plan involving all appropriate parties (client, physician, providers, employers, etc), identifies anticipated case results/outcomes, criteria for case closure, and promotes communication within all parties involved.Provide EHS service, solutions, programs, best practices and technical guidance to North American facilities and support functions to effectively and efficiently manage compliance assurance and EHSS risk mitigation.
