In lieu of case management experience, 3 or more years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions is required. Our more than 600 dedicated nurses, care coordinators, nurse practitioners (NPs) and business professionals integrate and manage care for more than 20,000 patients with late-stage chronic kidney disease (CKD) and end stage renal disease (ESRD) across the US each month.
Current, unrestricted license or certification to practice a health or human services discipline in the State of Oregon that allows the health professional to independently conduct an assessment as permitted within the scope of practice of the discipline (Licensed Mental Health Professional (LMHP) licensure such as Licensed Clinical Psychologist (LCP), Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Substance Abuse Treatment Practitioner (LSATP), Licensed Marriage and Family Therapist (LMFT) OR be a Qualified Mental Health Professional (QMHP)). Job Summary: The Case Manager of Behavioral Health will evaluate beneficiaries' needs, develop individualized care plans, and coordinates healthcare team members to ensure optimal client outcomes while adhering to payer requirements.
Ensure completion and signature of all required financial and admission-related documentation within 72 hours of admission, including but not limited to billing acknowledgments, payment plans, advance repayment agreements, coordination of benefits, authorized claims representative forms, and the initial Case Management Discharge Plan (CMDP). In addition, the role supports Case Management functions by ensuring compliance with State and Federal guidelines, participating in discharge planning, completing discharge needs assessments, coordinating aftercare services and appointments, and collaborating with the treatment team, payors, and facility leadership to support continuity of care.
