Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse.
Page 2. stay as they relate to reimbursement in New York State; thorough knowledge of required documentation for admission process and alterative level of care; good knowledge of medical case management planning, principles and techniques; good knowledge or electronic health records and electronic medical records; ability to perform, comprehend and interpret scores resulting from Patient Review Instrument; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications; sound professional judgment; capable of performing the essential functions of the position with or without reasonable accommodation. MINIMUM QUALIFICATIONS: Graduation from a regionally accredited or New York State registered college or university with a Masters degree in Nursing and three (3) years of acute hospital nursing experience, at least one (1) year in acute medical nursing; or: Graduation from a regionally accredited or New York State registered four (4) year college or university with a Bachelors degree in Nursing and five (5) years of acute hospital nursing experience, at least two (2) years in acute medical nursing; or: An equivalent combination of training and experience as defined by the limits of (A) and (B).
This role focuses on complex, high-utilization members with significant clinical, psychosocial, and transitional care needs to improve clinical outcomes, reduce avoidable readmissions, and ensure safe transitions across the continuum of care. The High Risk Nurse Case Manager collaborates with interdisciplinary teams, providers, facilities, and community resources to develop and implement comprehensive, member-centered care plans and ensure timely access to appropriate services.
p>Bilingual Case Manager (Move Coordinator) Key Responsibilities:
p>Reporting to the Supervising Clinical Case Manager and under the direction of the Clinical Director, the Case Manager is a dynamic, knowledgeable, and experienced individual with a social service, psychology, or clinical background to help make an impact on the ever-changing role of the court system and alternative initiatives aimed at reducing detention and crime rates in the city. Information collected about these arrestees and subsequent case processing is maintained in the agency's internal and externally shared database systems, which are used to conduct descriptive and evaluation research on arrestee characteristics, case processing and court outcomes, and on issues and potential reforms to criminal justice policy in New York City.
