p>The RN Case Manager demonstrates the knowledge, critical thinking, and judgment for nursing and case management practice according to the ANA Nursing Scope of Practice and nationally recognized case management practices according to the American Case Management Association. In collaboration with other clinical disciplines, this role integrates these evidence-based practices to facilitate optimal coordination of care, patient, family, and clinical staff communication and education, and optimal transition/discharge planning following CMS (Medicare's) Condition of Participation.
The purpose of the RN case manager to serve as case manager for Medicaid patients and help maintain stabilization, delay/prevent deterioration of health status, manage chronic conditions, and/or improve health status for patients while performing OASIS start of care, transfer, resumption, and discharge. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
li>Program Delivery: The Case Manager coordinates the timely collection and completion of required documentation for youth in care, ensuring that the youth on his/her caseload receives the full range of services identified in the ISP in a holistic manner. Assess strengths and level of functioning of children and families and determine their needs and appropriate treatment strategies using sound, objective decision making.
