The Transition Specialist will build a cooperative link between home, school, and community; will remove barriers that impede scholar growth and success; and will create roadmaps to ensure that every scholar is educated, self-directed, and a productive member of society. While performing the duties of this job, the employee is required to stand, walk, sit, use hands to finger, handle, or feel, reach with hands and arms, climb or balance, stoop, kneel, crouch, or crawl, talk, hear (in a quiet or noisy environment).
