The Lead APP partners with the Director, Manager, and Clinical Director for OSF OnCall Digital Health, and collaborates with physicians, advanced practice providers, and other care team members to drive service excellence, engagement, access, and cost-effective, efficient care delivery, including conducting regularly occurring meetings to facilitate information dissemination and team support. In addition to direct patient care, the Lead APP provides clinical leadership for a group of advanced practice nurses by reviewing and monitoring clinical quality in coordination with the Medical Director and Clinical Directors; supporting the design, implementation, and tracking of practice quality/outcome indicators; and ensuring consistent orientation and clinical practices across digital health programs.
