Key Responsibilities: Conduct comprehensive member assessments and evaluate healthcare and social service needs Develop, implement, and monitor individualized care plans Coordinate services among members, caregivers, healthcare providers, and community resources Manage a caseload of long-term care members and ensure ongoing engagement Conduct in-person home visits and telephonic follow-ups Authorize and coordinate referrals for healthcare and community-based services Monitor service delivery and identify gaps in care Collaborate with physicians, discharge planners, caregivers, and other stakeholders Assist members with complaints, appeals, and service-related concerns Maintain accurate and timely documentation within required systems Educate members and families on available benefits, services, and healthcare resourcesKey Requirements and Technology Experience: Skills-2 years of Care Management experience Strong field-based case management experience Long-Term Care Medicaid experience Medicaid and Medicare experience Experience managing large caseloads (50–70 members preferred) Home Health experience Experience working with Electronic Medical Records (EMR/EHR) systems Strong organizational and time-management skills Comfortable working independently in a fast-paced environment Discharge Planning Working with TruCare which is the software the team uses Education: Bachelor's Degree Required License: Valid Driver’s LicenseOur client is a leading Healthcare Industry and we are currently interviewing to fill this and other similar contract positions.
