Essential Functions: Assume responsibility, under the direction of the Director of Nursing and Executive Director, for the development and facilitation of resident-directed programming to meet the assessed needs, interests, and capabilities of the residents. Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable.
Summary of Position:The Nurse Case Manager coordinates resources and supports individuals who have sustained work-related injuries through assessment, care coordination, advocacy, and communication with healthcare providers, employers, payers, and other stakeholders. Research medical and community resources for individuals with catastrophic or chronic diagnoses, such as but not limited to AIDS, cancer, spinal cord injuries, diabetes, head injuries, back injuries, hand injuries, and burns, ensuring accessibility for individuals.
Working under the supervision of the Clinical Director, this non-clinical, non-patient-facing role focuses on facilitating communication, supporting care programs, and contributing to process improvement initiatives aligned with organizational goals. The role involves heavy use of computers, phones, and healthcare systems such as EMR/EHR platforms, along with frequent collaboration with internal teams, providers, and external partners.
Coordinates community placement for appropriate post-hospital discharge planning and coordination if patient needs a community placement such as a group home, sober living arrangement, Mental Health Nursing Home or other placement that would be deemed most appropriate to the patient's needs and safety. Required Licensure and Certification Master's level Licensure with the Behavioral Sciences Regulatory Board as one of the following: Licensed Professional Counselor (KS)Licensed Master Social Worker (KS)Licensed Marriage and Family Therapist (KS) .
Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse). Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.

Completes psychosocial assessments of patient/family situations including social, psychological, emotional, financial and other related factors to facilitate patients’ linkage to resources to support care in the community. Identifies and utilizes all relevant information (medical/nursing needs, social work knowledge base, disease process, knowledge of community resources) to accurately and thoroughly assess the patient’s psychosocial situation.
Required Education and Experience: Master's Degree Required Licensure and Certification: Master's level Licensure with the Behavioral Sciences Regulatory Board as one of the following: Licensed Professional Counselor (KS) Licensed Master Social Worker (KS) Licensed Marriage and Family Therapist (KS) State of Kansas Social Work license. Connects patient with Community Mental Health or other agencies or private providers to provide: Psychiatry follow-up appointments Psychotherapy and Expressive Arts therapy appointments, Intensive Case Management (Outpatient) Other social services such as substance abuse treatment, support groups (various subjects), etc.
