As a vital member of our Special Needs Plan (SNP) care team, the Care Coordinator (CC) is responsible for coordinating care for our members through close collaboration with the Care Manager, Social Worker, and other interdisciplinary team members.

Activities include: (a) determining inter- and intra-departmental dependencies; (b) directing and coordinating activities of project staff to ensure projects progress on schedule and within prescribed budgets and monitoring progress and modifying schedules as required; (c) ensuring that blueprints are followed and assisting with specifications for construction, related installations, major maintenance and special projects, and monitoring building contractors; (d) conferring with project staff to outline workplan and to assign duties, responsibilities, and scope of authority; and (e) conferring with project staff to provide technical advice and resolve problems.
