Responsibilities: EDUCATION AND PROFESSIONAL DEVELOPMENT - Coordinates, designs and implements educational programs to meet identified learning needs of staff, patients and families. Ensures appropriate and adequate clinical staffing and skill mix to meet patient care needs on own shift and upcoming shift.

Occupational Therapist II is a clinical specialist and subject matter expert in the diagnosis and intervention management of patients with various diagnoses, disabilities, and impairments affecting client factors (body function and structures), behavior patterns/roles/habits, and occupational performance.

li> Provides clinical assistance to determine appropriate services and supports due to member's health needs (including but not limited to: Prior Authorizations, Coordination with PCP and Specialty providers, Condition Management information and education, Medication management, Community Resources and supports). With compassionate attention and excellent communication, we collaborate with members, providers, and community organizations to address the full continuum of our members' health care and social determinant needs.
li>Processes referrals from members of the multidisciplinary team (social work, behavioral health, community resource coordinators, pharmacy, pharmacy technician, care managers) appropriately, accurately, and timely according to established workflows. Serving as an advocate for patients, the Care Coordinator will be part of the care management team and work in tandem with care managers, healthcare providers and community-based organizations to improve outcomes for patients they serve.
