Identify new referral opportunities by calling on hospitals, home health agencies, skilled nursing facilities, assisted living communities, clinics, and physician offices.
The Registered Nurse Care Manager provides case management services that are member-centric and include assessment, planning, facilitation, care coordination, evaluation and advocacy to all members across the healthcare continuum. Facilitate the coordination, communication and collaboration of the individual’s care among his/her providers including tertiary, non-plan providers and community resources with the goal of controlling costs and improving quality.
Champion post-discharge success by acting as a dedicated liaison between patients and post-acute services, ensuring a smooth, safe transition from hospital to home or specialized care. Optimize clinical and financial resources through strategic collaboration with payers and the care team, balancing top-tier patient advocacy with efficient healthcare utilization.
