The Care Coordinator will carry an assigned caseload and will deliver services singly or as part of a team and maintain accurate and timely clinical service records, prepare and provide accurate reports; and submit data in an accurate and timely manner. Requires advance knowledge of Microsoft Office Suite computer applications, aptitude for learning computer software applications and the ability to instruct others in the use of various software applications.
