p>Job Overview: Responsible for promoting appropriate Length of Stay (LOS) and resource management in areas assigned; assesses the care needs of patients upon entry into the organization and facilitates appropriate progression through the hospitals continuum of care into the post-hospital environment; collaborates with other members of the health care team in planning, coordinating, and facilitating the implementation of a plan of care based on the reason for admission and current, emerging, and projected care needs; performs these duties in a manner which enhances service excellence including quality clinical outcomes for patients, efficiencies in hospital operations, and cost-effectiveness; and demonstrates accountability for services provides through participation in the monitoring and evaluation of patient care activities.
