As a case manager, the RN will coordinate with an interdisciplinary team that includes the patients, families, physicians, medical assistants, social workers, counselors, and other Senior Care Center support staff to provide high quality care within the Senior Care Center model of a Patient Centered Medical Home (PCMH). The position will partner in identifying and achieving timely patient care needs with implementation of the plan of care that is appropriate, resourceful, and valuable as determined by the interdisciplinary care team, effectively utilizing tools that measure, monitor, and manage quality and stewardship of patient care resources.
